Great Plains Care at Home Network

The Great Plains Care at Home Network (GPCHN) is an emerging network of home health provider agencies working together to build a stronger, more collaborative future for care at home in Kansas. Guided by the experience and proven framework of the Ohio Community at Home Network (OCHN), GPCHN is being developed to create meaningful opportunities for Kansas agencies to share information, strengthen contracting opportunities, align on operational strategies, and address common challenges.

With the guidance of OCHN and a growing group of Kansas providers already engaged, GPCHN offers agencies the opportunity to be part of building a network designed specifically around the needs of Kansas home health providers. As the Network grows, so will the collective knowledge, resources, relationships, and opportunities available to its members.
