When selecting your membership level, please note the following:
Associate and Individual Memberships pay a flat rate.
Associate Members are official, regulatory, voluntary, and/or private organizations interested in the work of the Association, but do not provide direct patient services in the home.
Individuals are those persons interested in the promotion of home care and hospice in the state but would not be eligible for membership in another category or are not employees of a provider group.
All other categories listed are for In-Home Service Providers.
In-Home Service Providers. These are organizations whose primary business is the provision of health and/or support services which are delivered in the home. Membership rates are based on annual "Gross Revenue" which includes total charges for ALL services delivered in the home regardless of business type – home care, hospice, private duty, etc.
Note: Provider entities with different licensure numbers for multiple locations (ie. NOT branch offices) must use separate application forms for each location. Membership rights and benefits are non-transferable. They shall pertain only to the single licensed entity provided on this application. Misuse of association benefits may result in the loss of membership.
The Internal Revenue Code limits the amount of business
expense deductions for dues paid to an association that engages in lobbying activities even if
dues are not used for lobbying; the amount excluded for 2022 is 10% based on IRS criteria.
