Each year during our Annual Conference, the Kansas Home Care & Hospice Association likes to recognize some of the special people making a difference in the home care and hospice industry. During our 2025 Annual Conference in September, we will acknowledge individuals in the following categories:
 

Rising Star - This award honors a staff member of a home care or hospice agency in their first two to three years of employment in the industry who shows demonstrated growth and potential leadership, has taken on additional responsibility, and shows support for the agency and/or association. Nominees may come from any discipline. The KHCHA Awards Committee will select one recipient from the pool of nominees to be honored during the 2025 KHCHA Annual Conference this September.

Guiding Light - New to our KHCHA Awards in 2025, the Guiding Light Award is designed to recognize a seasoned professional in the home care or hospice field who exemplifies outstanding leadership, uplifts those around them, and fosters a positive, high-performing culture. This individual leads by example and makes a lasting impact through their commitment and influence. The KHCHA Awards Committee will select one recipient for this award from the pool of nominees.     

Guiding Light Nomination Form

Physician of the Year - This award honors a physician or nurse practitioner who excels in providing and utilizing home care and hospice, and who advocates for home care and hospice within the health care system. The KHCHA Awards Committee will select one recipient for this award from the pool of nominees. 

Physician Nomination Form

Home Care & Hospice Hero – This award is reserved for a friend of home care and hospice – someone not directly employed in the field, someone outside the home care and hospice agency arena and not an active board member.  Nominees may include volunteers, agency advisory board members, patients’ family members or friends. The KHCHA Awards Committee will select one recipient for this award from the pool of nominees.
