Session 303 Thursday, September 17, 2026 ~ 11:00 a.m. - Noon (CDT) 1.2 CNEs A Click of the Ruby Slippers: Navigating Hospice Live Discharges Description As more patients “graduate” from hospice, organizations face growing complexities in managing live discharge and care transitions. This session will explore best practices for transitioning patients from hospice, including how to evaluate equipment coverage during and after discharge to ensure continuity of care. Attendees will learn how thoughtful live discharge management can reduce risk, improve patient and family experience, and protect operational performance. The discussion will also highlight how effective transition processes support stronger Star Ratings and enhance overall hospice reputation.
