A Click of the Ruby Slippers: Navigating Hospice Live Discharges


Description

As more patients “graduate” from hospice, organizations face growing complexities in managing live discharge and care transitions. This session will explore best practices for transitioning patients from hospice, including how to evaluate equipment coverage during and after discharge to ensure continuity of care. Attendees will learn how thoughtful live discharge management can reduce risk, improve patient and family experience, and protect operational performance. The discussion will also highlight how effective transition processes support stronger Star Ratings and enhance overall hospice reputation.

Kristin Nelson is Senior Vice President of Post Acute Sales at Dragonfly Health, overseeing national sales efforts across the Hospice, Pharmacy and PACE divisions. With over 20 years of healthcare experience, she has played a key role in strategic growth, implementation, client engagement initiatives and leads the team for disaster preparedness response. Prior to joining Dragonfly Health, Kristin held business development and implementation roles at Enclara Pharmacia. She holds a B.S. in Biochemistry and Health Psychology and an MBA in Healthcare Administration.


Continuing Education

Kansas Home Care & Hospice Association is approved as a provider of CNE by the Kansas State Board of Nursing. This course offering is approved for 1.2 contact hours applicable for APRN, RN, or LPN relicensure. Kansas State Board of Nursing provider number: LT0287-0314. 


Other disciplines will need to apply independently – a certificate of attendance for 1.0 CE hours will be provided.
