Description Rehospitalizations remain a significant challenge in home health care, impacting patient outcomes, quality measures, and healthcare costs. This session will explore evidence-based strategies that home health clinicians can implement to identify high-risk patients, address preventable causes of hospitalization, and improve care coordination across the continuum. Participants will learn practical approaches to patient assessment, medication management, disease-specific interventions, caregiver engagement, and timely communication with physicians and interdisciplinary team members. Emphasis will be placed on proactive care planning, early recognition of clinical changes, and effective use of home health visits to support patients in safely remaining at home. Continuing Education Kansas Home Care & Hospice Association is approved as a provider of CNE by the Kansas State Board of Nursing. This course offering is approved for 1.2 contact hours applicable for APRN, RN, or LPN relicensure. Kansas State Board of Nursing provider number: LT0287-0314.
