Description

Social Determinants of Health impact outcomes are a current focus of data collection for CMS. This session will explore current data trends, the SDOH items in OASIS E1, proposed SDOH items slated for 2027 inclusion, and why this data is being collected. Learn how developing the plan of care with patient’s health related social needs in mind can improve your agency’s quality and VBP outcomes.

Jennifer Osburn is a well-versed, nationally known home health consultant and educator. Having a diverse background that includes extensive field staff, administration, and technology/software experience allows opportunities for her to establish a unique, “real-world” connection with home health professionals of all disciplines. Jennifer’s 29 years of clinical experience, combined with an ability to engage her audience, has led to numerous speaking engagement invitations across the nation, including for The Joint Commission and ACHC. She holds certifications for Home Health Diagnosis Coding and OASIS specialist.

Continuing Education

Kansas Home Care & Hospice Association is approved as a provider of CNE by the Kansas State Board of Nursing. This course offering is approved for 1.2 contact hours applicable for APRN, RN, or LPN relicensure. Kansas State Board of Nursing provider number: LT0287-0314.
