Follow the Yellow Brick Road to Survey Readiness: A Home Health Compliance Journey

There’s no place like home—and for surveyors, there’s no grace at home. With condition-level deficiencies on the rise and enforcement remedies tightening, every home health agency must be survey-ready 365 days a year—and shouldn’t leave the outcome to luck. From a clinician’s perspective, this session pulls back the curtain on the federal survey process: the structure of the Conditions of Participation (42 CFR Part 484), the standard, abbreviated, extended, and complaint surveys defined in 42 CFR Part 488, and the redesigned seven-task survey flow. Drawing on real Form CMS-2567 findings and the most-cited national and state deficiencies, Annette Lee will show what surveyors look for in the clinical record, where agencies stumble, and how to build a survey readiness binder, a whole-team game plan, and the QAPI-driven culture that keeps your agency on the right side of the yellow brick road. Leaders will leave with a practical, year-round framework they can put to work on Monday morning.


Target Audience

Home health agency administrators, directors of nursing and clinical services, clinical managers and care coordinators, QAPI and compliance staff, and agency owners—from small rural agencies to large urban providers. The session is designed for registered nurses and other clinical leaders responsible for survey preparation, regulatory compliance, and quality oversight, and is appropriate for those new to the survey process as well as experienced leaders seeking a current refresher.

A registered nurse with more than three decades of experience in home health and hospice—spanning direct care, clinical leadership, and service as a hospice Director of Nursing—Annette pairs deep bedside insight with command of the Conditions of Participation, OASIS, coding, survey readiness, and quality reporting. A sought-after speaker and educator, she partners with state associations and agencies across the country to translate complex CMS requirements into practical, clinician-friendly strategies that protect compliance and strengthen patient care.
