Challenging Topics in Hospice Pharmacy --  

Part 1:  Preventing the Prescribing Cascade at End of Life 

Part 2:  Facing the Challenges of Anticoagulants in Hospice


Description

This program will address two challenging topics in hospice care.  In part 1, we will discuss the prescribing cascade, in which a new medication is prescribed to “treat” an adverse reaction to another medication in the mistaken belief that the reaction is actually a new condition or symptom. We will discuss strategies for recognizing whether new symptoms may actually be medication side effects, for deprescribing nonessential medications, and for better managing end of life medication use for the hospice and palliative care patient.

In part 2, we will talk about how hospices and palliative care organizations are often faced with the challenge of admitting patients who are prescribed anticoagulants, often for diagnoses that are only indirectly related to the terminal diagnosis. We will review the outcomes data and risk/benefit profile of these medications to help hospice staff and clinicians make informed, data-driven decisions about continuing or deprescribing these medications for patients in hospice and palliative care.

Caren Martin, PharmD, BCGP

Senior Clinical Manager, Enclara Pharmacia, Philadelphia, PA

Dr. Martin earned her Doctor of Pharmacy degree from the University of Nebraska Medical Center and completed an executive residency with the American Society of Consultant Pharmacists. She has enjoyed working in a wide range of nonconventional clinical roles, including pharmacy association management, senior care, pharmacy benefits management, ambulatory care, value-based care, and hospice. She is a board certified geriatric pharmacist, has worked as a North Carolina Clinical Pharmacist Practitioner,  and has authored more than a hundred articles for national pharmacy publications.  She is a frequent presenter at state and national hospice conferences.  Dr. Martin is currently serving in the role of Senior Clinical Manager at Enclara Pharmacia.


Continuing Education

Kansas Home Care & Hospice Association is approved as a provider of CNE by the Kansas State Board of Nursing. This course offering is approved for 3.0 contact hours applicable for APRN, RN, or LPN relicensure.  Kansas State Board of Nursing provider number:  LT0287-0314. 


Other disciplines will need to apply independently – a certificate of attendance for 2.5 CE hours will be provided.
