    Tuesdays, April 17, April 24, May 1 and May 8, 2018
    1:00 to 2:30 pm Central

Recordings Available through May 31, 2018

    Webinar 1

    Patient Centered, Interdisciplinary Care Planning

    Tuesday, April 17, 2018 * 1:00 – 2:30 p.m. CST

Webinar Overview:  The stated principles for transforming the CoP include using a patient-centered, interdisciplinary approach that recognizes the contributions of various skilled professionals and their interactions with each other to meet the patient's needs.  This session will be exploring core competencies that all clinicians should possess, regardless of their discipline, to meet the needs of the 21st-century health system: provide patient-centered care; work in interdisciplinary teams; employ evidence-based practice; apply quality improvement, and; utilize informatics. The session will continue with strategies for interdisciplinary collaboration and how they can result in better outcomes of patient-centered care.

Objectives:  After attending this webinar, the participant will be able to:

    1. Describe the required standards of 484.50(c) and 484.60 in the new CoPs
    2. Review best practice strategies to incorporate into agency care planning
    3. Outline a structure and process that operationalizes these standards

Target Audience:  Home health administrators, clinical managers, and clinicians

    Webinar 2

    An Integrated, Living QAPI Program

    Tuesday, April 24, 2018 * 1:00 – 2:30 p.m. CST

Webinar Overview:  484.50(c), 484.60 and 484.65 in the new CoPs can be operationalized within a structure and process that integrates these standards within an agency cost efficiently.  This session will present strategies that objectify processes in practice and documentation to address these requirements. Care plan outcomes are targeted through an embedded QAPI program that loops back around with carefully constructed Plans of Action intended to influence targeted outcomes before they occur. This session will also outline the metrics to monitor for evidence of successful outcomes and the documentation necessary for evidence of compliance with the new CoPs.

Objectives:  After attending this webinar, the participant will be able to:

    1. Describe the required standards of 484. 65 – Quality Assessment and Performance Improvement (QAPI)
    2. Outline a structure and process that operationalizes these standards in an integrated method
    3. List metrics quantifying improvement in outcomes and lowered costs with this approach

Target Audience:  Home health administrators, clinical managers, and clinicians

    Webinar 3

Changes in Therapy Approach to Home Health

    Tuesday, May 1, 2018 * 1:00 – 2:30 p.m. CST

Webinar Overview:  CMS plans changes to the PPS case mix system “to rely on patient characteristics to set payment for therapy and non-therapy services and should no longer use the number of therapy visits as a payment factor.” The practice patterns and therapy compensation creep associated with the current PPS methodology will fit poorly with a case mix system not relying on a therapy visit number. However, practice pattern changes do not occur as abruptly as payment changes.

This session will review the proposed new Home Health Groupings Model and the impact on therapy practice. We will then apply reasonable and necessary plans of care to typical home health conditions and link them to the outcomes agencies strive to achieve. Participants will learn to prepare changes in practice and to demonstrate the value therapy services can bring to the home health agency.

Objectives:  After attending this webinar, the participant will be able to:

    1. Describe the changes in the Proposed Groupings Model for HH PPS
    2. Review the impact on therapy from eliminating thresholds in the PPS case mix
    3. Explain how reasonable and necessary therapy plans of care should directly relate to outcomes home health agencies need

    Target Audience:  Clinical managers, Clinicians

    Webinar 4

The Value of Remote Patient Monitoring to Outcomes

    Tuesday, May 8, 2018 * 1:00 – 2:30 p.m. CST

Webinar Overview:  CMS recently pledged more support for telehealth in upcoming payment rules. Longstanding advocates have complained about the lack of coverage for telehealth and the impact it has on the home health industry. The focus will be on how to leverage technology to improve access to care. The 2018 physician fee schedule included new billing codes for telehealth, including chronic care management and data analysis collected by remote monitoring. Regardless of specific coverage for remote patient monitoring, there is great value in the service for improved outcomes and reduced cost. This webinar provides insights into one remote patient monitoring call center. We will review the relationship of the call center with the agency, its nurses, the patients, and the value it brings to improved patient outcomes.  Karen Vance will be joined by Karen Thomas from Advanced Telehealth for this webinar.

Objectives:  After attending this webinar, the participant will be able to:

    1. Explain the history of telehealth’s role in home health to date and recent CMS announcements for coverage
    2. Describe the difference between telehealth and remote patient monitoring
    3. Define the relationships between the call center and the agency
    4. List metrics quantifying improvement in outcomes and lowered costs with this approach

    Target Audience:  Administrators, Clinical managers

    Reducing Hospital Re-Admissions

    Webinar

    Wednesday, May 9, 2018
    12:00 –  1:30 pm CST **

** Recording available for those unable to participate live.

Overview:  Changes to reimbursement and funding has meant reducing acute care hospitalizations isn’t just good for your patients, it’s critical to your agency’s bottom line as well. Hospitalization is extremely expensive, accounting for one third of the $2 trillion in total annual health spending in the U.S. last year.  Perhaps surprisingly, as many as one in nine hospital admissions is a re-hospitalization – when a patient is readmitted to hospital within 30 days. For the elderly, re-hospitalization can be traumatizing, yet it might be avoidable. Learn proactive solutions to improve your readmission rates using the PDSA approach.

    Objectives:

    • Define and utilize a PDSA approach
    • Identify the primary causes of readmissions in your agency
    • Develop a Performance Improvement Project that will fit into the new Quality management & COP
    • Discuss the process to be used including data sources and education components
    • Apply this information to achieve both positive change and compliance with the new QM process

    Presenter:

Kathy Roby, BSN, MA, MS, CHCE, Senior Project Manager, Qualidigm.  Kathy is the Consulting Director at Qualidigm, which is New England’s QIN-QIO.  She has lectured nationally on transformational change in the home healthcare setting.  Roby has assisted large and small agencies develop strategic initiatives to improve quality, safety, and cost-effective service.

    Continuing Education:

The presenters and program planners have no conflict of interest regarding this webinar. RNs and LPNs must participate in the entire presentation for contact hours to be awarded.  To apply for nursing contact hours, please e-mail a typed list, and attach an evaluation form and a completed sign-in sheet, listing the individuals at your facility that participated and noting those requesting contact hours.

Individuals requesting contact hours will receive a certificate of contact hours earned. Please make sure that you enter the Contact Person’s name in the Registration Form. All correspondence and site registration information will be sent to this person. No commercial support has been provided for this program.

Convenience:  Enjoy the convenience and cost-efficiency of a webinar – watch the speaker’s slide presentation on the internet while listening by telephone or through your computer’s microphone and speakers (VoIP).  There is no limit to the number of attendees from your agency who may participate at your site using one phone line and a computer with internet access.

Confirmation: Prior to the webinar, a GoToWebinar link will be e-mailed to you. You will need to register through this link to attend the webinar. You will be sent a confirmation which will include a join link to access the webinar, a dial-in number and an access code to listen in via telephone. You will also be sent any pertinent handouts if available.

Registration Fee: Registration fees are based on one phone/webinar connection; multiple site participation for your agency will require a separate registration fee for each connection. Please note: if your agency has sent in 1 registration but 3 people from your agency register through the GTW link, your agency will be invoiced for the 2 additional registrations.  Following the webinar, you will receive a link to access the recording.
