    Hospice Item Set (HIS) Updates

    Hospices have two new quality measures to begin collecting data for on April 1, 2017 - Hospice Visits When Death is Imminent Measure Pair and the Hospice and Palliative Care Composite Process Measure.  The HIS tool has been updated to reflect the new reporting requirements and hospices will begin using the HIS version 2.00.0 with admissions and discharges after April 1.  A Hospice Compare website and public reporting is on schedule for implementation in summer 2017.  To utilize standardized tools most successfully, it is important to understand the instructions for each component of the HIS and the measure definitions.  

    This informative webinar will:

    • Review the current HIS reporting requirements
    • List the two new quality measure details
    • Explain the four new HIS items
    • Provide a preview of publicly reported data
    • Share common HIS questions and concerns while providing tips for dealing with these issues

    Presenters:  Katie Wehri, BSBA, CHC, CHPC, is the hospice specialist with Ohio Council for Home Care & Hospice, has an extensive background in regulatory, licensing/certification and accreditation standards interpretation and implementation.  Certified in healthcare compliance (CHC) and healthcare privacy compliance (CHPC) through the Health Care Compliance Association, she also works in hospice operations with the National Association for Home Care & Hospice.  Katie has been consulting nationally in these areas since 2009.

    Amy Hoyda, RN, BSN, MBA, CHPN, is the Director of Quality Assurance/Performance Improvement/Education at Bridge Home Health & Hospice in Findlay, OH.  Her career positions include post-acute care setting in various roles. She has experience to apply healthcare quality measure management and lean process improvement methodologies in the post-acute care setting with a special focus on educating about quality integration and lean application to both management and front line staff.  

    Continuing Education:  The presenters and program planners have no conflict of interest in regard to this webinar.  RNs and LPNs must participate in the entire presentation in order for contact hours to be awarded.  To apply for nursing contact hours, please mail an evaluation form and a $10 processing fee for each individual requesting hours.  Additionally, send in a completed sign-in sheet, listing the individuals at your facility that participated and noting those requesting contact hours. 

    Individuals requesting contact hours will receive a certificate of contact hours earned.  Please make sure that you enter the Contact Person’s name in the Registration Form.  All correspondence and site registration information will be sent to this person.  No commercial support has been provided for this program.

    Ohio Council for Home Care and Hospice is an approved provider of continuing nursing education by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.  (OBN-001-91) (OH -157, 4/1/2019).

    Confirmation: A confirmation will be sent via email to verify site registration, as will the handouts, Internet link and 800 number for entering the webinar, and instructions for participation.  If your Contact Person does not receive the E-mail confirmation two (2) business days prior to the webinar, please contact our office to verify your status.

    Registration Fee per site/phone connection: Registration fees are for one (1) phone connection.  SHARING OF REGISTRATION OR FEES WITH OTHER AGENCIES AND/OR INDIVIDUALS IS PROHIBITED.  The live webinar includes the opportunity to ask questions of the presenter.  Contact hours may be earned for listening to the audio recording up to 6 months following the live webinar.  If you desire to purchase both the live webinar and the audio recording, pay the registration fee and an additional $30.

    Understanding Mental Health Disorders

    Description: This presentation will explore some mental health disorders that may create challenges for the Nurse Aide in the home setting. According to the National Alliance on Mental Illness approximately 43.8 million adults or 1 in 5 adults in the U.S experiences mental illness annually.  Chances are that some of the patients we care for in the home may be experiencing some sort of mental illness.  Learn strategies that can help you when caring for these patients.

    Presenter: The presenter for this activity is Cindy R. Morgan, RN, MSN, CHC, CHPN, Associate Vice President of Hospice, Palliative Care and Clinical Innovations, AHHC of North Carolina. Cindy is a certified trainer in coaching supervision and also a certified Health Coach. She has worked in various director level positions in Home Care and Hospice throughout the years. Her work in Hospice began as a volunteer nurse in the 1980’s. Later in her career, she was the Assistant Vice President of a large Home Care and Hospice organization in Central North Carolina where she was responsible for overseeing both home health and hospice operations. Cindy has served as a Board member for the NC Center for Nursing and is a past member of the NC Board of Nursing.

You will be sent a confirmation which will include a join link to access the webinar, a dial-in number and an access code to listen in via telephone. You will also be sent any pertinent handouts, if available, an evaluation and certificate. The webinar will take place from 3:00 p.m. to 4:00 p.m. EST, 2:00 to 3:00 pm CST.

    Registration: The registration deadline is December 7, 2016.  Registrations may not be shared between agencies – the agency’s registration covers the access of only one phone line/computer access to the webinar. Multiple site participation for your agency will require a separate registration fee for each connection. Please note: if your agency has sent in 1 registration but 3 people from your agency register through the GTW link, your agency will be invoiced for the additional 2 registrations. The handouts will be emailed to you to the email address you provide. Please feel free to provide an additional email address as a backup. 

    Registration Fees:

    Non-Members:  $190 per webinar access

    Recording also available.  If unable to participate live, registrants will have 30 days to access the recording.

    Cancellations:  Fees will be refunded only if written cancellation is received one week prior to the workshop and no refunds after the webinar link is sent to your agency. In the event of a written cancellation, KHCA will retain, or charge, $25 of the initial registration fee, per registrant, to cover administrative overhead.

    A Team Approach to 
    Medication Management

    Webinar – Tuesday, December 13, 2016

    Medication management is a key component of quality home health care.  Medication related issues are a leading cause of hospitalization/ re-hospitalization for all patients, but especially for older patients.

    Several Care Transition Coordination Demonstration Projects have significantly decreased hospital re-admissions, reduced health care cost and decreased mortality of community dwelling patients through improved medication education and management of medications. Home health staff are in the perfect position to positively impact medication management because we are in the homes of our patients and can readily recognize concerns and issues as well as significantly impact better medication management.

    Home Health Value Based Purchasing, Star Ratings, Home Health Compare and IMPACT have all targeted aspects of medication management as key outcomes as part of their quality initiatives. In addition, medication related deficiencies are among the top cited issues in home health certification surveys.

    It takes the entire home health team working together to address this vital aspect of health care to make a difference and improve the quality of care for those we serve every day. This 90 minute webinar is devoted to promoting a Team Approach to Medication Management in home health. The program will include:

    • An overview of projects that have had a significant impact on improving patient’s life quality while reducing health care costs and other negative consequences of inadequate medication management.
    • Discuss the accurate scoring of the OASIS items related to medication management to ensure that home health agencies capture credit for their contributions to superior outcomes related to medication management.
    • Identify strategies to improve medication management, with special focus on the drug regimen review and focused interventions that have proven effective in decreasing medication related issues.
    • Identify basic actions each member of the home health team can take to improve the overall success of medication management and the benefits to our patients.

    Presenter: Judy Adams, RN, BSN, HCS-D, AHIMA Approved ICD-10-CM Trainer, Adams Home Care Consulting, Inc. is a well- known health care consultant and speaker with over 30 years of healthcare experience and extensive experience in home care and home health management. Judy frequently speaks nationally on home health coverage; coding; HH COPs; PPS and medical review.
