KHCA awards honor individuals who have exhibited distinguished performance in an extenuating circumstance. To be considered, nominations must be specific to exceptional performance in one or more particular situations at a particular time.

Because we know that distinguished performance occurs year-round, award nominations are accepted throughout the year.  KHCA will select an honoree for each award category to recognize at the KHCA Annual Meeting each September. The deadline to be considered for this year's awards is June 20, 2008. The 2008 awards will be presented on Wednesday, September 17, 2008, at the Wichita Marriott Hotel in Wichita, Kansas. 

Below are the nomination forms and criteria for the five KHCA awards (in PDF format). Please feel free to make copies of the forms and distribute them throughout your agency, and to those outside your agency, to encourage nominations in all categories. You may nominate as many in each category as you wish, and individuals may be nominated more than once.

• Home Care Administrator Honoree of the Year (This may be any management level individual in a home care agency – Director, Administrator, Case Manager, etc.)

• Home Care Professional Honoree of the Year (This could be any state-licensed individual who works primarily in home care – RN, LPN, PT, OT, COTA, CPTA, etc.)

• Home Care Paraprofessional Honoree of the Year (This could be any state-certified individual who works in a paraprofessional capacity in a home care agency.)

• Helpful Hint Honoree Award (Agency that enters the most innovative strategy or helpful hint/pointer.)

• Home Care Hero Honoree of the Year (For a "friend of home care" who is not actually employed in the field.)
